neglect, and utilization.

Obtain information
contained in his/her medical record
within a reasonable time frame when
requested.

Be free from
chemical and physical restraints
that are not medically necessary

Complaint and give
suggestion either orally or in
writing. These complaints should be
addressed.

Receive from the
physician an explanation of the
reasons for transferring to other
hospital and give the receiving
hospital all necessary reports and
results of tests and investigations.
